You can get this process started by providing us with your patient history by
completing our CLIENT PREPARATION FORM. |

We require that you bring in your old eyeglasses so that we can inspect them for prescription,
bifocal height, pupliary distance, lens material and design, and the fitting of the frame. We need
to understand all the likes and dislikes about your previous eyeglasses in order to determine how
much improvement in your vision you can expect from your new Vision Fore Golfing eyeglasses. |

With your patient history on file, we will be able to provide you with all the best and
up-to-date information and products when you make return visits to us. As we are compiling
your patient history we will fill out your work order, review your pricing and payment options,
and process your claim to any qualifying vision care plan. |
